1. For Spouse Applicants
• Death Certificate of Member-Consumer/ Barangay Certification provided by ILECO III signed by Punong Barangay
• Marriage Contract
• Proof of Ownership – (e.g. deed of sale, affidavit of heirship) (if available)
2. For Legal Heirs
• Death Certificate of Member-Consumer/ Barangay Certification provided by ILECO III
• Notarized Waiver of Rights stating that the other successors are not after on the said connection.
• Proof of Ownership – (e.g. deed of sale, affidavit of heirship) (if available)
B. IF MEMBER-CONSUMER IS NO LONGER THE OWNER OF THE PROPERTY
• Proof of Ownership – (e.g. deed of sale, affidavit of heirship) (if available)
• Notarized Waiver of Rights and Interest (if available)
• Barangay Certification provided by ILECO III signed by Punong Barangay
C. ADDITIONAL REQUIREMENTS FOR NEW MEMBER-CONSUMER
• Attendance to Pre-Membership Seminar
• Valid IDs as proof of identity (preferably government-issued ID like TIN)
• 2×2 ID Picture
• Membership Form provided by ILECO III
• Payment of relevant fees amounting to P 285.00
Upon completing the said requirements, you may submit it to the nearest area office.
