2. For Legal Heirs
• Death Certificate of Member-Consumer/ Barangay
Certification provided by ILECO III
• Notarized Waiver of Rights stating that the other
successors are not after on the said connection.
• Proof of Ownership – (e.g. deed of sale, affidavit of heirship) (if available)
MEMBER-CONSUMER IS NO LONGER THE OWNER OF THE PROPERTY
• Proof of Ownership – (e.g. deed of sale, affidavit of
heirship) (if available)
• Notarized Waiver of Rights and Interest (if available)
• Barangay Certification provided by ILECO III signed by Punong Barangay
ADDITIONAL REQUIREMENTS FOR NEW MEMBER-CONSUMER
• Attendance to Pre-Membership Seminar (temporarily
waived due to COVID-19 pandemic)
• Valid ID’s to prove identity (preferably TIN ID)
• 2×2 ID Picture
• Membership Form provided by ILECO III
• Payment of Relevant Fees amounting to P 285.00
Upon completing the said requirements, you may submit it to the nearest area office.
